Meet Our Leadership Team

BRIAN BANKS – Owner/CEO

In 2012 Brian acquired Providence Home Care and Providence Private Duty, strategic companies providing health care services to our aging population. In 2022 Brian led the expansion of Providence services by adding a third business line, Hospice. He is integrally involved in the daily operations of his businesses. Brian holds a Bachelor of Business Administration from the University of Oklahoma and a Master of Business Administration from Oklahoma City University. The best part of Brian is Vickey, his wife of 36 years. At their home church in Oklahoma City, they regularly teach young couples on the biblical principles for marriage and family. Brian and Vickey have two married children with three grandchildren. Brian loves to go to the mountains where he enjoys fly-fishing.

KERN CREEVEY – Chief Operating Officer

Kern partnered with Providence in 2022 and oversees the operations and the strategic growth of all three Providence businesses.  He possesses 16 years of leadership experience in the healthcare industry, ranging from start-up healthcare technology businesses, small independent healthcare organizations, to large corporate health systems, across the care continuum.  Kern has always been drawn to the senior care space as he sees it to be such a meaningful calling and understands the impact a mission-driven business, like Providence, can have on an aging population.  He holds a BA from the University of Notre Dame and MHA from the University of Saint Francis.  In addition, he continues to be active in the community sitting on three non-profit boards and is involved coaching youth sports. Kern is married with four children.

CHRYSTAL MOHR, RN, BSN – Director of Quality and Compliance

Chrystal graduated from OU College of Nursing in 1995 with a Bachelor of Science in Nursing. In 2002 she started her career in home health care as an RN Case Manager.  Chrystal was soon promoted to Clinical Coordinator.  Since that time, she has gained experience as a Director of Nursing, Director of Quality Assurance and has earned Oasis Specialist Certifications in COS-B and COS-B as well as her Home Care Coding Certification/HSC-D.  Chrystal currently holds her license as a Home Health Administrator.

BECKY RUMMAGE – Director of Personal Care

Becky was drawn to the elderly from an early age. She has served the seniors around her in various capacities. She has cared for close elderly friends, acted as a resource for family caregivers and the aging in her community, and has served in faith-based nursing home ministries. She has a heart for seniors and a desire to grant their wish to age in place with independence and dignity. Her professional history includes working as a Care Manager for seniors on the Advantage Waiver Program, aging Veterans, and Private Pay clients. She has a heart for professional and family caregivers, desiring to see them healthy, thriving, and equipped with the resources available. Called to others, Becky has a passion for serving in various community projects and church ministries.

SHERI MULLIKIN – Director of Corporate Care

Sherí Mullikin is our Corporate Care Director who leads Everybody Matters initiatives, promotes our core values and company culture, and manages key relationships in the Oklahoma City area. Since she started at Providence in 2015 she has passionately pursued excellence in the way we serve our teammates, patients, and referral partners. Sherí believes every person is valuable and deserves our best care. She considers it a privilege to work alongside highly skilled and compassionate teammates to accomplish the mission and vision of Providence Home Care. Previously, Sherí served in church ministry for 16 years, focused on leading people to build relationships in community. She loves connecting with people, coaching, and is passionate about international community development.  

SUE GIBSON – Director of Operations

Sue began her nursing career at Comanche County Memorial in Lawton and followed her military husband on moves to Germany, California, and Oklahoma over 20 years. She worked ICU, Orthopedics, Psych, and Emergency Department before finding Home Health.  This became her passion, working as Supervisor, Director and Regional Director in Home Health care for the past 30+ years.  Home Health became very personal for her over the years, as her own family has a rare form of Muscular Dystrophy and she became the advocate for her father and 3 siblings who were all impacted.  This has heightened her understanding of patient and family needs and the level of help that is required.  She feels that it is a privilege to care for patients in their homes and be a part of their lives and able to impact their care.  Providence provides this high level of quality, compassionate care through our outstanding staff.  

LaBERTA BAKER, RN – Hospice Administrator

LaBerta Baker joined Providence Home Care in August of 2021 as the Hospice Administrator for Hospice Services.

LaBerta is a Registered Nurse and holds Administrator Licenses for Home Care & RCAL/Assisted Living. With years of Administration in Home Care & Hospice she finds great joy in Hospice Care. Hospice Care is a calling and a gift that is given to those chosen nurses, she feels honored to receive and accept the calling. LaBerta has had the opportunity to Lead & Supervise Nurses in a couple of States during the Covid Pandemic, which made a huge impact in the lives of many. With her knowledge and expertise, we’re looking forward to providing Hospice Care in a manner of Excellence and Compassion to all patients and families that we encounter.

LaQUIETTA COLLINS, RN – Director of Hospice Clinical Services

LaQuietta Collins has worked as a hospice nurse for the past 24 years. Her passion has always been to inspire excellence in the delivery of end of life care. She is honored to be called into the most sacred and vulnerable time in peoples’ lives. LaQuietta finds joy in participating in deeply meaningful conversations with patients and ensuring that their symptoms are managed. “Hospice care is my gift, my ministry and I am grateful for the privilege to care for patients and families at the end of life.” LaQuietta is a Registered Nurse with a B.S. in Organizational Leadership. Currently, she is the program chair for the Oklahoma Hospice and Palliative Nurses Association and the former program chair for Agape Road, an end of life care home.

TIM IHRIG, MD – Hospice Medical Director

Dr. Ihrig is a renowned global expert in hospice and palliative care, known for his commitment to promoting complete transparency in patient care. Dr. Ihrig obtained his Doctor of Medicine and M.A. in Molecular Biology from the University of Nebraska, and he has received recognition as a clinician-educator by the Center to Advance Palliative Care. We are honored to have Dr. Ihrig’s expertise and dedication as part of our team at Providence Home Care.

His compelling TED Talk, titled “What We Can Do to Die Well,” passionately encourages physicians to prioritize the overall quality of life while providing compassionate support to individuals facing serious illnesses at the end of their lives.
